What is Hydrodissection

Hydrodissection is a minimally invasive, ultrasound-guided injection technique that involves the precise delivery of fluid—typically a combination of sterile saline and a small amount of local anaesthetic to gently separate and release soft tissue structures that are adhered or entrapped. At The Injury Hub, we specialise in using hydrodissection for Achilles and Patellar tendinopathy/tendinosis, conditions frequently seen in runners and athletes.
Unlike steroid injections, no corticosteroids or regenerative medications are used, making hydrodissection an appealing option for patients seeking natural, low-risk pain relief and restoration of movement.

How Does It Work?

Under real-time ultrasound guidance, fluid is carefully injected into the space between the painful tendon and the surrounding Kager’s fat pad (Achilles) or Hoffa’s fat pad (knee). These regions are often infiltrated by neovessels (new blood vessels) and nerve ingrowth, which are strongly associated with chronic pain.

By mechanically “stripping” or dissecting the tendon away from these inflamed adjacent structures, the procedure:

  • • Reduces abnormal nerve input that perpetuates pain

  • • Disrupts neovascularisation, which often maintains chronic inflammation

  • • Restores natural tendon gliding and mobility

  • • Creates space for proper tissue remodelling and repair

This technique does not damage the tendon instead, it facilitates recovery by restoring the natural interface between tissues.
